(P9) Impact of joint cervical cancer screening and HIV follow-up care on adherence to cervical cancer screening guidelines in women living with HIV

Abstract

Background
The ethiological agent of cervical cancer (CC) is persistent infection with one or more oncogenic HPV genotypes. Screening and early diagnosis is essential for systematically CC prevention. Women living with HIV (WLWH) have a higher incidence and genotype distribution of high-risk HPV infections compared to women in the general population (WGP), which emphasizes the importance of cancer preventive screening amongst WLWH. Danish study data shows that less than half of Danish WLWH followed the recommended CC screening guidelines for WLWH, though an additional group following the screening recommandations for the general population. As a consequence, in 2014, Capital Region of Denmark established an initiative offering CC screening, in combination with the annual HIV control for WLWH.

Objectives
The purpose of this study is to evaluate a combined clinical HIV control and CC screening with respect to increasing participation in CC screening among WLWH. We evaluate the differences in demographic characteristics, HIV data and cervical cytological abnormalities detection rate and grades between 1) WLWH who did attend the embedded screening offer 2) did not attend the embedded screening offer, 3) other WLWH in Denmark, and 4) WGP.

Method
WLWH attending the Department of Infectious Diseases, Copenhagen University Hospital, Hvidovre were invited to CC screening in the outpatient clinic embedded into the regular HIV control. The gynaecological examination was performed by a medical doctor from the Department of Gynaecology.
CC screening in Denmark targets women from 23-64 years. Information on screening participation and results were obtained from the Danish Pathology Database (Patobank). There were no exclusion criteria.
Data on HIV demographics and combined antiretroviral therapy will be retrieved from The Danish HIV Cohort Study; a nationwide, prospective, population-based cohort of people living with HIV who are followed at one of Denmark’s eight HIV centers.
Screening compliance and identification of predictors will be analysed by logistic regression analysis. Descriptive statistics will be applied, continuous variables anlysed by ANOVA and categorical variables by Chi Square test or Fisher’s exact test. P-value <0.05 will be considered statistically significant for all analyses and will be performed using SAS 9.4 .
